City High School Tax Credit Pledge Form

This pledge form is designed to allow donors to indicate exactly where they want their dollars to go and to update contact information. This form is designed to accompany an on-line contribution or a mailed check. Please send tax credit donation checks to City High School, 48 E. Pennington St., Tucson, AZ 85701 OR follow up with an on-line contribution through our secure Pay Pal link.
    The maximum one can claim as part of the Arizona State School Tax Credit Program each year is $400 (filing jointly) or $200 (filing as an individual).
